What is home health care?

Home Health is a type of medical practice that delivers care to individuals in the comfort of their own homes. This practice focuses on providing medical treatment and therapy services to patients recovering from illness, injury, or those with chronic health conditions.

Who qualifies for home health care?

To be eligible for Home Health Care, a patient must have a physician's determination of needing skilled nursing and/or therapy services, and be homebound during the length of authorized care. This service is available to individuals of all ages.

What services are offered?

We offer a wide range of services for both home health, and hospice patients including skilled nursing, physical therapy, occupational therapy, speech therapy, pain management, counseling, respite care, and so much more. What is hospice care?

Hospice care is a range of services that focus on providing comfort and quality of life for patients with a terminal illness who are nearing end of life. It emphasizes quality of life through pain and symptom management, as well as emotional and spiritual support for the patient and their family

Who is eligible for hospice?

Patients are eligible for the use of their hospice benefit upon a terminal diagnosis being provided by their physician. The patient must also agree to forgo curative treatment in favor of comfort care. See our Hospice page or more details.

How can hospice care help?

Hospice provides an interdisciplinary team of professionals including nurses, doctors, social workers, chaplains, home health aides, and volunteers. Does hospice mean giving up hope?

No, hospice care does not mean giving up hope. It focuses on maximizing quality of life by providing comfort and support during the various stages of a terminal illness. Patients quality of life often improves due to the level of care being provided by their hospice care team.
